User ID: The User ID is a unique identification name that you create. The User ID must be from 6 to 20 characters in length. The User ID must begin with a letter or number.
Password: The Password is a password that you create. The password must be from 7 to 28 characters in length. Click on the "Password Rules" link for more information on creating a password.
Confirm Password: The password entered here must be the same password you enter in the Password field.
First Name: Enter your first name here.
Middle Name: Enter your middle name. This is optional.
Last Name: Enter your last name here.
SSN: Enter your Social Security number here.
Date of Birth: Enter your date of birth.
Zip Code/Postal Code: Enter your current home address zip code or if your current address is a foreign address enter your Postal Code.
Phone Number: Enter a valid 10-digit phone number where you can be reached. Before you can use your OMS account, a PERS representative will contact you at the phone number provided to verify your identity over the phone.
Email: Enter the email address you would like to use.
Confirm Email: The email address entered here must be the same email address that you enter in the Email field.
